To Err Is Human—named after a 1999 Institute of Medicine report estimating hospital mistakes killed nearly 98,000 in the U.S.—is to be screened at large hospitals and major healthcare organizations across the country. This fall, the film will be released to the public.

More recently, a 2015 report from the U.S. Centers for Disease Control and Prevention (CDC) blames medical errors for 440,000 American deaths per year. This would make a medical error the number-three cause of death in America—behind only cancer and heart disease. Over five years, the CDC report notes the following:
- 2,413 operations on the incorrect body part
- 4,857 surgical instruments left inside a patient
- 27 operations performed on the wrong patient
They also state that nearly 1.7 million people a year acquire harmful infections while staying in the hospital—and note that perhaps 70% of these were preventable by diligent hand-washing or other simple precautions.
